Below are the forms that you will need for YLC 2009.  For your benefit they are all listed in two forms, but they are identical in content.

Banner Instructions:

This year each parish will be asked to make their own banner for YLC.  You will need to have your parish name (city) and the theme on it, along with how your youth see the Spirit impower them..

Youth Minister's Registration Booklet: 

This document gives you information as youth minister to help get ready for YLC, it includes a schedule and workshop choices.

To help you advertise this event we are providing a flyer to hand out.

Individual Registration Form:

Everyone of the participants (Youth, Adult or College Age) need to fill out this form, so that it is easier for you as youth minister to put them on to the on-line registration.

Youth Permission and Release Form:

Each High School participant needs to fill out this form, you keep the orginial and send a copy into the youth ministry office with registration.  Please check all signatures.

Adult Chaperone Form:

Each adult chaperone must fill out this form, you will need to send in a copy of both pages to the youth ministry office.

Adult Medical Form:

Each adult chaperone must fill out this form, you will need to send in a copy to the youth ministry office.

Collegiate Leadership Conference Application Form:

Each participant currently in college must fill out this form, you will need to send in a copy to the young adult ministry office.

Collegiate Young Adult Guidelines:

Each collegiate participant over age 18 must fill out this form, you will need to send in a copy to the young adult ministry office.

Collegiate Leadership Conference Flyer:

Background Check Form: 

This is a parish form that indicates that all your adult chaperones have gone through Virtus and have had a background check.  One form for the whole group and must be sent into the youth ministry office.
