Youth Leadership Conference Forms

Below are the forms that you will need for Youth Leadership Conference 2012.  For your benefit they are all listed in two forms, but they are identical in content.

This document gives you all the information you need to set up the Youth Leadership Event for your parish, along with all of the forms

This document include the information and forms each group will need when handing out materials.  Some editing for your parish information will be needed.

Below is each form that you would need for this event, that were also found in the booklet. Most forms are in word and pdf to make it easy for you to down load which ever version works best for your computer.  Some are only available in one form.  

Parish Registration Form:  (word)    (pdf)

Use this form as your cover sheet for everytime you send in your registration packet.

Youth Registration Form:  (word)   (pdf)

All youth from incoming Freshman to Graduating Seniors that are participanting in the Youth Leadership Conference, so that it is easier for you as youth minister to put them on to the on-line registration. A copy needs to be sent in with your registration packet.  This years registration form for youth has the workshops on it.

Youth Permission Form:   (word)   (pdf)  

Each High School participant needs to fill out this form, you keep the orginial and send a copy into the youth ministry office with registration.  Please check signatures.

Chaperone Policy Form:   (word)  (pdf)

Each adult chaperone must sign this form, you will need to send in a copy to the youth ministry office.

Adult Registration & Medical Form:  (word)   (pdf)

Each adult chaperone must fill out this form, you will need to send in a copy to the youth ministry office. 

Virtus & Background Check Form:  (word)   (pdf)

This is a parish form that indicates that all your adult chaperones have gone through Virtus and have had a background check.  One form for the whole group and must be sent into the youth ministry office.  Please send in when all chaperones are listed.

Choir Application:  (word)  (pdf)

if you have participants that would like to be part of the choir, please have them fill out this application.  We ask that choir members have attended one YLC before being in the choir.

Conference Assistants (CA) Application: (word)  (pdf)

This form is for those who are in college that would like to serve as part of the team for the conference.  Formly known as Staffers, this is a new title we are using.  Please have those interested send in this application to the Youth Ministry office by April 30th.

Add/Drop Form:  (word)   (pdf)

This form is used for making changes to those already registered.
